Rhiannon is currently in the ICU receiving therapeutic leukocytapheresis. The goal is to achieve prompt leukocyte reduction in her blood and prevent leukostatic syndrome which causes tissue hypoxia (organ damage).
She presented to the hospital with chest pains early in the morning on Thursday. She said “my heart is failing me”. One of the effects of leukostasis is tumor lysis syndrome where the cellular contents of the malignant cells are released into her circulatory system. If she were given chemo right now, this could happen because it would blast these malignant cells to pieces and their contents would enter her blood stream at a high concentration because of her counts. The purines released by these cells turn into uric acid and cause toxic effects, one of which is heart failure.
The leukocytapheresis is an emergency measure to hopefully avoid the tumor lysis syndrome so she can receive chemotherapy and ultimately a bone marrow transplant to save her life.
So far, it is working! Her WBC count dropped from 73 to 54 (probably lower now) and her vitals have stabilized. She is, however, in severe pain.
